REGISTRATIONS are NOW CLOSED

If you are interested in registering, please email us at pvsoccer@gmail.com.

We will not be accepting additional registrations, unless we need to complete an age group.
We will make contact with you and a late fee of $25 will apply.

FALL REC Registrations will take place in June, 2024.

Cal North/US Club Competitive Soccer Forms

All Coaches, Assistant Coaches, Trainers and Managers must fill out the Cal North Risk Management Registration Form (LIVESCAN) - Once you complete the online background you MUST print out the form and take it to one of the locations: Santa Cruz Livescan, 877 Cedar St, Suite 150, Santa Cruz - 831-621-5041 - Monday - Friday 9 - 6p - FREE

If you DO NOT go to the above approved Livescan location and choose to go to the below location:
Watsonville: UPS store, 1961 Main St, Watsonville - M-F 9am - 5:30pm, Sat 9am - 4pm
You MUST take the printed form from the Cal North site along with the instructions on the second page of this document.

Refund information

All refunds must be requested in writing (No refunds on Scholarships)

Requests received by March 1st or August 1st (as below) will be refunded minus the $35 processing fee.
